Sheltering Arms is seeking a Program Coordinator for our Developmental Disabilities Services in the Bronx. The Program Coordinator of Residential Services will serve as the QIDP of these residences, in addition to providing direct supervision of 2-3 Residence Managers and ensures the efficient coordination of supports and services to individuals residing in the Individualized Residential Alternative (IRA) setting, in which the administrative team maintains a high quality program in accordance with the mission statement and standards of the Office for People with Developmental Disabilities and Sheltering Arms Children and Family Services., i.e.: Individualized Residential Alternative “IRA” homes, personalized supports, and person-centered planning. The Program Coordinator must be able to effectively supervise/ lead, support, motivate, provide training, exercise patience, work with a flexible schedule, and respond to a variety of situations in the event of an emergency with professionalism, skill and tact. Ingenuity and creativity must be utilized when determining the best training methods, assistance, and supports provided to the management team. Overall program compliance and quality of service/care must be the foundation and core of service provision to those we serve in the residential programs.

 

Essential Functions:

  • Provides direct supervision of 2-3 Residence Managers as assigned.
  • Ensures that the Residence Managers build a support network between the Individuals, direct support staff and community members.
  • Prepare and review monthly habilitation service data documents for Individuals and ensure the timely submitting for billing. 
  • Prepares progress reports for Individuals immediate support group/team, including their family and/or guardian, staff, employment specialist, case manager, and other professionals.
  • Ensures consistent service delivery that connects work, home and recreational needs.
  • Serves as an active member of each Individual’s Inter-Disciplinary Team (IDT), which includes the Individual Habilitation Plan development.
  • Submits monthly written reports to the appropriate managers outlining operational status, progress, and concerns.
  • May perform the duties of the Residential Program Manager as necessary, in accordance with current staffing needs.
  • Presents problems and recommends any necessary changes in current policy and procedures to the Associate Director and/or Director of DD Services.
  • Monitors financial, medical, personal and Individual Habilitation Plan (IHP) documentation to ensure accuracy.
  • Supports Residence Manager and Associate Director with the interviewing and hiring of job applicants.
  • Supports Residence Manager with the completion of Performance Reviews for the direct care workers.
  • Lead/participate in treatment team meetings/conferences for future planning and the development of goals/valued outcomes.
  • Monitors and reports health and medical needs of individuals in collaboration with the management and clinical team.
  • Provides 24/hr on-call monitoring and support to the assigned residential programs.
  • All other duties as requested by the Associate Director of Residential Services.

Qualifications:

  • BA in Human Services or related field required
  • Minimum of 3 years of direct experience with the OPWDD population.
  • Ability to judge and supervise active treatment issues
  • Ability to develop, review, and monitor program goals.
  • Training in AMAP, First Aid, CPR and SCIP-R
  • Certified QIDP
  • Proficiency in Microsoft Office Suite and program-related software.
